You will find the Member Information Sheet under the Questionnaire section.  You may print it and return it by snail mail, email or drop it in the drop box at the Rec Center (the box will be installed on Monday, November 12th).  The box will have a lock on it so no one will be able to get your information.  Thank you for your cooperation.
